• *Photo of your  I.d/ Driver's license/ Passport (you can cross out your address, I just need your name and picture) 

or

• Email sent from your employee email address

• Linked in Profile

This is non-negotiable and for my safety. Information collected for screening will be disposed upon verification and is never stored. Your privacy is of the utmost importance to me.

If a same-day booking, please make sure you indicate this and make sure all information needed is submitted.

References are recommended but aren't required. They will just make you look better! If submitting, the rules are: 

2 recent (within the past 6 months) provider references along with their websites.
